Project ECHO® Hypertension and Cardiovascular Disease Interdisciplinary ECHO Series
Founded on the principle of “All Teach, All Learn,” Project ECHO® has evolved into a globally recognized model for collaborative learning and capacity building. Inspired by the medical rounds integral to a clinician’s residency, Project ECHO® creates an interactive environment where all participants including specialists, community practitioners, and support teams share knowledge, insights, and experiences.
As an extension of the Project ECHO® model, the Hypertension and Cardiovascular Disease Interdisciplinary ECHO Series advances evidence-based best practices for hypertension and cardiovascular disease management. This free, interactive series addresses health-related social needs, strengthens community-clinical linkages, and improves patient outcomes through a team-based, interdisciplinary approach.
What to Expect from Our ECHO Session?
- Monthly sessions are held virtually on the fourth Thursday of each month from 12PM-1PM EST.
- Each 60-minute session consists of short introductions from Subject Matter Experts, a short didactic presentation, followed by a case presentation and group discussion by all participants.
- ECHO sessions are carefully designed by faculty, facilitators, and other health care and public health professionals to address the knowledge needs of healthcare teams, from clinicians to community health workers.
- All patient information is de-identified to ensure HIPAA compliance during patient presentations and case reviews.
- All sessions are recorded for future viewing. Participants can access them via the SharePoint link provided upon registration or on Constellation Quality Health’s YouTube Channel.
Intended Participants
Our focus is supporting populations at risk for hypertension and cardiovascular health disparities. We welcome participation from multidisciplinary care team members, public health practitioners, and community-based organizations across the Southeast region of the United States, particularly in North Carolina, South Carolina, Georgia, and Florida.

Have you encountered unique challenges with patients at risk for or managing hypertension and cardiovascular disease? Do you wish you could consult an expert or collaborate with other professionals on a particular patient case?
We encourage you to submit potential cases. Any case is valuable for discussion as we learn and grow together in this “All teach, All learn” environment.
